Case Management


The Community Support Program

The role of case management is to assist people with mental illness to achieve independence – that is, the ability to live in the community with the least intervention from formal services and, to the greatest extent possible, to make their own decisions (including decisions about receiving case management services). Participation in the Community Support Program (or CSP) is voluntary, goal driven, and participant directed.

CSP services are provided in a flexible manner to meet a participant's changing wants, needs and resources. To the greatest degree possible, they are provided in the environment of your choice.

Case managers also help obtain required services by assisting with access to a full range of mental health and/or other community resources.

Case managers provide support to help individuals learn more about, and ultimately make decisions regarding:

  • Developing and promoting independence
  • Education
  • Employment / Volunteering
  • Life and social skills development
  • Health awareness
  • Linking with other community services

Specialized Community Support Services

Complex Needs Case Management

The Complex Needs Case Management component of The Community Support Program is intended to assist individuals who experience both a diagnosed or suspected mental health issue and a cognitive impairment such as a brain injury or a developmental handicap (dual diagnosed).

This component of the program also provides client oriented consultation services to community agencies and organizations who work directly or indirectly with these individuals.

In the interest of promoting education to the community and our partners in service provision, informational seminars or presentations are available as requested and presented on site.


Concurrent Case Management

The Concurrent Case Management component of The Community Support Program is designed to assist individuals who have both a diagnosed or suspected mental health issue and substance abuse (Concurrent Disorder).

This program also provides consultation services to community agencies and organizations who work with people with a concurrent issue.

To promote education and awareness to both the community and service providers, presentations are available as requested and presented on site.


Urgent Needs Case Management

The Urgent Needs Case Management component of The Community Support Program is designed to assist individuals diagnosed with or suspected of having a mental health issue with short-term client centered case management. Depending on the needs of the individual they will be seen promptly in their community.

This service does not take the place of immediate crisis services.
